Two Cases of Cushing¢¥s Syndrome Caused by Adrenocortical Adenoma







Abstract
Two cases of Cushing¢¥s syndrome caused by adrenocortical adenoma were presented.
A 27-year old female was admitted to Yong san hospital, Chung-Ang University on April. 9, 1987 because of gradual weight gain, plethoric moon face, injected conjunctiva, and pretibial pitting edema (case 1).
A 43-year old female was admitted to Chung-Ang University Hospital on Jun. 22, 1987 because of exertional dyspnea. chest tightness, palpitation, headache and hypertension (case 2).
On physical examination the younger female (case 1) was moderately developed with moderate hirsutism, buffalo hump, hypertension, plethoric legs, emotional depression and had a history of irregular menstruation.
The older female (case 2) was moderately developed with plethoric moon face, buffalo hump and truncal obesity.
Laboratory tests revealed increased 24 hrs urinary excretion of 17-OHCS and free cortisol and elevated plasma cortisol levels in both cases.
Urinary excretion of 17-OHCS or free cortisol and plasma cortisol levels were not suppressed by low and high dose dexamethasone suppression tests in both cases.
The value of 24hrs urinary excretion of 17-KS and plasma ACTH were within normal limits and were not suppressed by high dose dexamethasone suppression test in both cases.
On adrenal C-T study, about 3.2 x 2.6 x 2.4 cm sized, well defined, smooth bordered, ovoid shaped mass was noted in right adrenal gland
(case 1) and about 2.5 x 2.0 x 2.0 cm sized, spherical shaped,
